medical (non-cosmetic) indications for laser hair removal

Laser hair removal can be performed for aesthetic and comfort reasons.

However, we regularly receive requests for laser hair removal for medical reasons, the most common of which are:

1. Recurrent ingrown hairs after waxing, shaving, etc.  (bikini line, underarms and even legs

2. Keratosis pilaris (very common) (small rough red spots on the back of the arms)

3. Keloid scars on ingrown hairs on black skin (use only YAG laser! - no diode, flash lamp, etc.

4. Pilonidal sinus (plastic surgeons refer patients to us for hair removal from the intergluteal cleft after surgery)

5. Hirsutism (abnormal hair growth on the face and below the navel in women due to hormonal disorders)

Recurrent folliculitis:

Recurrent inflammation and infection after each hair removal session using another method (waxing, shaving, etc.)

1. Pilonidal sinus: Diode laser, Nd-Yag LP, Alexandrite LP, 4 sessions/2-week intervals, pain/local anaesthesia (topical, local), no complications, no recurrence: 86.6% (Follow-up: 5-7 years).

 

2.Hidradenitis suppurativa: chronic disease that progresses in flare-ups. The lasers offered are Nd-Yag LP, diode, and IPL. 20 patients: biopsies before and after treatment with Nd-Yag LP (deep penetration), parameters: 10mm, 20-35 days, double staking (1st treatment), triple staking (2nd treatment) / Inflammatory lesions. Improvement: 2nd month (32% fibrosis), 4th month (clinical and histological 72.7%). Treatment must be aggressive: Staking+++

 

3. Dissecting cellulitis of the scalp: Chronic inflammation (papules, pustules) with scarring alopecia. Diode laser monotherapy, 4 sessions, tolerance+++, no recurrence/6 months-1 year.

 

4. Pseudofolliculitis barbae: high demand, Nd-Yag LP laser/safe and effective method, reduction/papules and pustules & size/hair, 50%/1 session, 90%/3 months High fluences are recommended: 50J/cm² (PT IV), 100 J/cm² (PT V-VI); Reduction: 56%/3 sessions. Some authors recommend low doses: 12 J/cm2, 2-5 weekly sessions. However, the Alexandrite LP laser causes worsening of the condition.

5. Keloid acne on the neck: laser treatment should be the first treatment offered. It is effective and has a low recurrence rate. The recommended lasers are Diode, Alexandrite LP, Nd-Yag LP and Er:YAG. Reduction of 90-97% after 4 sessions.

 

6. Trichostasis spinulosa: Hair removal laser treatment provides a permanent solution by destroying the telogen hairs trapped in the same keratin sheath and responsible for hyperkeratosis. The treatment is reported to be effective with minimal discomfort and no side effects. No recurrence in 90% of cases (follow-up: 2 years).

 

7. Keratosis pilaris: common, affects the proximal parts of the limbs, responsible for erythema and rough skin. Short wavelength vascular lasers reduce erythema but not skin irregularity. Significant improvement in skin texture and roughness was achieved after 3 sessions of 810 nm diode laser treatment over 4-5 weeks.

 

8. Axillary hyperhidrosis: Nd-YAG LP: 1 session/month, treated side & untreated side, biopsies: destruction of eccrine glands on the side treated with Nd-Y AG LP. No improvement with 800 nm diode laser.

7. Becker's nevus: Laser treatment is not satisfactory; treating hypertrichosis and hyperpigmentation remains challenging, with a risk of blistering. Diode or Nd YAG LP lasers are used at low fluences/5 sessions over 6 weeks; improvement noted after 1 year.

8. Skin grafts: involving hair (surgical reconstruction/basal cell carcinomas of the nose, burns to the fingertips, etc.)

 

9. Reconstruction of intraoral structures (pharynx and hypopharynx, etc.): skin grafts with hair in the oropharynx cause irritation, hypersalivation and food trapping. It is difficult to insert the laser into the oral cavity. However, the Alexandrite LP (7 mm) has been used successfully through the laryngoscope light at 90°.

10. Laser hair removal from the intravaginal and intraurethral areas of transsexuals:

vaginoplasty and phalloplasty

 

11. Urethral repairs (hypospadias): The use of a 15W diode laser with a lateral beam in the postoperative period allows hair to be treated. Hair promotes the formation of stones.

 

12. Hair growth around stomas: prevents the adhesion of equipment and shaving and causes folliculitis.

13. Frontal hair transplantation in women: Unsightly thick hair can be treated with laser. Nd-YAG LP (35-36J/cm2, 6 ms) reduces hair diameter without damaging the grafted follicles (improvement/87%).
